At Hygieia Complementary Therapies, we take your right to privacy seriously. We only accept and hold the information from you relevant to the types of services/appointments you have enquired about or agreed to provide you with the best possible service and meet our insurance requirements.
When you first enquire, we will only contact you to provide the relevant information and confirm any appointments or training you require. This will be through your preferred method of contact in which you have chosen to initially contact us or an alternative agreed form of communication, which may have been discussed or details you have provided.
During your appointment or training, you will be asked to complete a contact and consent form outlining our privacy policy, including the official opt-in options. This will confirm your preferred contact methods and provide you with complete control of how you would like to be contacted. Any consent can be revoked at any time if you so choose.
We will never pass on your details to a 3rd party unless we have your consent (e.g. if we have recommended another professional and you have expressed the desire for them to contact you directly).
Personal information must be kept for seven years post your most recent treatment. However, you have access to these files at any time. We only keep information that our insurance providers require.
If you are an expectant Mom, we must keep records for seven years post the 18th anniversary of your baby's EDD.
If we treat a minor between the ages of 16-18, the records must be kept seven years after their 18th birthday.
If we treat anyone aged 16 or under with the traditional parental consent and attendance during therapy, we will also be required to keep records for seven years post their 18th birthday.

The type of information we are required to hold for therapies and training includes:

Name and some form of contact information

DOB and lifestyle information

Health information, including medication and medical history

Treatment plans and reflections
-
Any other information is relevant to the therapy or training.
If you see a student therapist at the centre, the centre will keep your contact details securely (as usual); however, you are provided with a reference number that the learners will be aware of. The learners will also keep a treatment plan with only your reference number but no other personal details; this is for their training requirements and Treatment evidence. However, your identity will be kept separate from their notes.
All records are kept securely in locked cabinets and password-protected devices and software.
